As the parent/ legal guardian of the stated minor, I give my expressed consent to obtain counseling from Anchored Virtual and the stated counselor. I understand that Anchored Virtual encourages parents to remain present for counseling sessions. While the counselor’s highest priority is to provide excellent help to the minor, the outcome of counseling is dependent on many things including the participation of the minor client, parents and other support systems as well as the counselor. Therefore, I will not hold Anchored Virtual or the counselor responsible for the outcome of counseling.
